Custom Service Agreement

This Custom Service Agreement ("Agreement") is made between LCIW, LLC, offering the Coach & Trainer Development Program under Conscious Uprising ("Business"), and the enrolled participant ("Client"). By continuing with this program, the Client acknowledges full understanding and agreement with the terms outlined below.

1. Program Services
The Business will provide the Client with a custom CDTP experience, which may include but is not limited to:

• 1:1 coaching sessions tailored to leadership, facilitation, and transformation work.

• Group training intensives (virtual and/or in-person)

• Practice facilitation labs and peer coaching

• Assignments, exercises, and live feedback

• Access to digital learning resources and recorded materials

• Optional certification pathway upon completion

The structure and frequency of services may be customized based on the Client’s development path and agreed scope of services.

2. Tuition and Payment

• Program tuition will be outlined in the Client’s enrollment confirmation and agreed upon prior to starting the program.

• Payments will be processed using the method on file or per a payment plan agreed upon in writing.

• All tuition payments are non-refundable.

• If the Client withdraws before completion, they remain responsible for a prorated portion of the fees based on services rendered, plus a $50 administrative cancellation fee.

3. Dispute Resolution
Any disputes will be governed by the applicable laws of the State of Florida.

Payment Disputes
If a payment is cancelled or reversed, the Business reserves the right to terminate services and pursue payment for services rendered at a rate of $400/hour. If legal action is taken, the Client will be responsible for any associated fees and costs incurred by the Business, including legal and administrative time at a rate of $500/hour.

4. Acceleration Clause
If the Client fails to make timely payments or attempts early termination without fulfilling financial obligations, all future payments become immediately due and payable. The Business may charge the Client’s payment method on file for the full outstanding amount.

5. Responsibility & Assumption of Risk
The Client acknowledges that personal and professional development work may involve emotional processing, perspective shifts, and lifestyle changes. By participating, the Client assumes all risks and releases the Business from liability, except in cases of gross negligence.

6. Governing Law
This Agreement shall be governed by the laws of the State of Florida.

7. No Guarantees
While the Business provides tools, mentorship, and training to support transformation, it does not guarantee specific results. Testimonials or case studies are illustrative and not promises of outcomes.

8. Entire Agreement
This document represents the full agreement between the Client and the Business. No verbal or implied agreements shall supersede it. Modifications must be made in writing and mutually agreed upon.

9. Severability
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full effect.

10. Licensing & Authority
The Business affirms it holds any required licenses to deliver these services and that all facilitators or coaches are qualified to provide the training outlined.

11. Termination & Completion
This Agreement will remain in effect until the end of the program unless terminated earlier by mutual agreement. If terminated early, the Client is responsible for payment of all work completed up to the date of termination.

12. Acknowledgment of Agreement
By proceeding, the Client confirms:
